Job Overview – RN Field Case Manager (Day Shift):

Compensation: $56/hour

Location: Montgomery County, PA

Schedule: Monday to Friday, 9:00 AM – 5:30 PM

Our healthcare client is hiring an RN Field Case Manager (Day Shift) in Montgomery County, PA to deliver community-based nursing care and case management for individuals with intellectual and developmental disabilities (IDD). In this hybrid role, you’ll provide clinical services such as wound care, vitals, and medication administration while coordinating care with clinics, physicians, and residential programs. Ideal for a licensed RN with field experience and a passion for person-centered care.

Responsibilities as the RN Field Case Manager:

  • In-Home Nursing Care: Provide direct care to group homes daily, including assessments, vitals, wound care, and medication administration.
  • Care Coordination: Manage clinical care plans, documentation, and serve as the nursing point of contact for assigned homes and programs.
  • Program Oversight: Support medical needs at the day program site, including medication management and routine assessments.
  • Clinic & Provider Coordination: Schedule and coordinate physician visits and clinic appointments through the program center.
  • Documentation & Fieldwork: Maintain accurate nursing records per state guidelines while balancing 40% field visits with 60% office-based coordination.

Qualifications for the RN Field Case Manager:

  • Licensure: Active Registered Nurse (RN) license in the State of Pennsylvania required.
  • Experience: 2+ years of field-based case management and clinical nursing experience, with strong preference for backgrounds in IDD, hospice, home care, or long-term care.
  • Technical Skills: Proficient in computer systems with moderate to high typing speed for documentation.
  • Skills & Attributes: Compassionate, dependable, and collaborative, with a strong interest in supporting individuals with developmental and physical disabilities.
  • Transportation: Must have a valid PA driver’s license and a reliable vehicle (mileage reimbursement provided); daily travel between sites is required.
