Position: Registered Nurse RN - Home Health

Job Type: Full-Time (FTE)

Location: West Reading-PA

Base Salary: $72,000 to $95,000 +Best-in-class benefits

Responsibilities:

*Under the general supervision of the Home Health Clinical Manager, provides professional nursing services to individuals in their home through a process of nursing assessment, planning, care and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures and the Nurse Practice Act.

*Carries primary case management functions for assigned patients.

Education Requirements:

* Graduate of an accredited school of Nursing

Certifications & Licensures:

* RN license in the State of PA

* Current Valid Driver's License

* Current Automobile Insurance

* BLS Certification

Candidate Details:

*1+ to 2 years experience

*Minimum Education - Bachelor's Degree
