Responsibilities

• Deliver comprehensive skilled nursing care and manage patient cases.
• Provide support and care during extensive treatment and end-of-life stages.
• Identify additional patient care service needs (internal and external), make appropriate referrals, and coordinate overall patient care.
• Communicate admission information and patient condition changes to the Patient Care Coordinator (PCC) and implement agreed-upon plan of care adjustments.
• Provide timely and relevant patient and family education and support based on diagnosis and anticipated outcomes.
• Participate in quality assessment and improvement initiatives.
• Offer professional guidance and support to LPNs to foster their clinical growth and excellence.
• Maintain a positive attitude during periods of change and support all organizational activities and processes that benefit the VNA Family of Services.
  
  

Qualifications

• Active PA RN License
• Valid PA Driver's License and reliable car
• Minimum 6 months of experience as an RN (any setting)
